Ministry to Minors Application
The following MS Word document is a required application for all adults working with any minor during a Southwest Community Church sponsored event or ministry. The form is for children's ministry workers, teachers, and leaders. The form is also for BIN volunteers, leaders and staff. The document may be downloaded, saved to your computer, completed and emailed back to us at staff@southwestonline.org.
